BENEFITS OF BEING A MEMBER

  • Collectively become business owners, influence and participate in the management of frontline health services.

  • Children under 18 residing with a user member benefit from the same benefits free of charge.

  • Benefit from preferential rates for services not covered by the RAMQ or for professional health services other than those provided by doctors.

How much does it cost to become a member?
You must acquire 5 shares (total of $50) per adult. This amount is refundable upon departure from the Coop according to the terms provided for in the law on cooperatives.

A tax receipt will be issued for donations of $100 or more.

Having a family doctor

It is important to specify that the mission of the Coop santé is not to guarantee a family doctor.
The mission is to facilitate access to local health care for permanent residents of Wentworth-Nord and the surrounding area.

For the moment the clinic is not yet operational and we cannot offer any services, however if you do not have a family doctor:

IMPORTANT

I undertake to respect the regulations of the Cooperative and I declare to use the services offered by the Cooperative. By completing the membership form and purchasing shares, I request to be admitted as a member of Coop  de solidarité santé Wentworth-Nord. ​

 

COST OF ADMISSION TO THE COOPERATIVE

I understand that the purchase of qualifying shares allows me to be a member of the Cooperative. If I no longer wish to be a member, I can request reimbursement of my qualifying shares. These will be reimbursed to me in accordance with the Cooperatives Act and according to the financial capacity of the Cooperative. Upon my death or if I fail to request reimbursement in writing, I will donate my shares to the Cooperative.​

ACCESS TO A FAMILY DOCTOR

I understand that the Cooperative must allow any person residing in Quebec, members and non-members, to access the medical services provided by the Régie de l'assurance santé du Québec under the same terms and upon simple presentation of a health card. valid health insurance.

PROTECTION OF PERSONAL INFORMATION: Coop  de solidarité santé Wentworth-Nord undertakes, without time limit, to maintain the strictest confidentiality concerning personal information.
